Findings: On March 20, 2025, Alleged Victim (AV) was found in Witness 1's (W1) room in W1's bed. AV was observed being hit repeatedly by W1 which resulted in AV suffering a skin tear on AV's left forearm. Based on facility documentation and interviews, AV has history wandering into other residents' rooms and lays on their beds to sleep. AV's care plan does not address AV wandering into other residents' room. The investigation determined, the facility failed to implement interventions and appropriately care plan related to AV's known behaviors. The facility's failure resulted in a physical altercation with W1, causing unreasonable discomfort to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
