Findings: On or about March 25, 2025, the facility failed to appropriately monitor Witness 1 according to his/her known behavior and prior resident-to-resident altercations. Based on facility documentation and interviews, at time of incident, the facility failed to provide supervision for safety resulting in W1 punching Alleged Victim (AV) in the face causing injury and unreasonable discomfort to the AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00645 $169.00 fine assessed
