Findings: The AV relies on the facility to provide a safe environment. Witness 1 (W1) has a history of behaviors and has multiple altercations with staff and residents. W1 has care planned interventions for when agitation is noted. According to an investigation, on or about April 2, 2025, W1 was digging their nails into the AVs had, causing unreasonable discomfort and requiring staff intervention. Staff identified W1 was noted to be agitate prior to the incident and no interventions were implemented to ensure a safe environment for the AV, which is a violation of resident rights, is considered neglect of care, and constitutes abuse.
