Findings: On or about April 1, 2025, the Alleged Victim (AV) was found by staff, shaving his/her body parts in his/her room. AV's razor is typically locked up for AV's safety, and it is unknown how AV obtained the razor. Due to AV having access to this razor, and shaving him/herself, AV suffered razor burn as a result. The facility's failure to ensure a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
