Findings: The Alleged Victim (AV) relies on the facility for his/her care. Witness 1 (W1) has a history of resident-to-resident altercations and the service plan documented a history of recent physically aggressive behaviors. W1's service plan did not include effective interventions for staff to provide when W1 was having aggressive behaviors. According to an investigation, on or about April 02, 2025, AV was heard yelling and was found on the floor after W1 reportedly threw AV to the floor, resulting in a skin tear to AV's elbow. The facility failed to properly address resident behaviors, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
