Findings: Witness 1 (W1) has a history of domestic violence towards Alleged Victim (AV) and AV's adult children. On or about April 2, 2025, AV had bruising on AV's right eye and jaw that were unknown in origin. AV had bruising and swelling to AV's forehead. Witness 2 (W2) claimed that AV fell. W1 was yelling at AV to get up and AV was visibly shaking. AV also had a scratch on AV's shoulder. On or about April 7, 2025, W1 was witnessed raising W1's hand like W1 was going to strike AV. On or about April 19, 2025, W1 was observed hitting AV's right shoulder. AV was crying after W1 hit AV. The facility failed to provide a safe environment to AV, which resulted in AV sustaining injuries from W1, which is a violation of resident rights, is neglect of care and constitutes abuse.
