Findings: Alleged Victim (AV) lives at Respondent’s facility and relies on facility staff for all AV's safety and care needs. AV is a known fall risk, including incidents that occurred in March 2025, April 2025, and May 2025. The facility failed to develop sufficient interventions to mitigate the risks to AV. On or about May 15, 2025, facility staff found AV lying on the floor from an unwitnessed fall in the common area resulting in head pain, and a bruise on the back. EMS transported AV to the emergency room for evaluation. Respondent failed to develop and implement suitable interventions. This constitutes abuse by neglect.
