- Findings
- Alleged Victim (AV) had an active order for the narcotic medication. On or about May 1, 2025, Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) disposed of AV's unused narcotic medication card containing 30 pills. AV's narcotic medication card had been incorrectly labeled for destruction by unknow AP1 staff. Medications are not supposed to be destroyed if the resident still has an active order. It was the facilities best practice to destroy extra narcotics. AP1 could not provide medication disposal policy/training prior to the May 1, 2025, incident. AV's insurance would not cover another refill, so the facility paid to have AV's medication refilled. The allegation that AP2 and AP3 failed to provide a safe medication administration system by wrongfully destroying AV medication without permission was investigated, and wrongdoing was not substantiated. The facility failed to provide a safe medication administration system which is a violation of resident rights, is considered neglect of care and constitutes abuse.
