Findings: According to the documentation, the facility failed to follow the service plan for the Alleged Victim’s (AV) known fall risk. The AV is a known fall risk and has interventions in place to prevent falls, including the use of a tab alarm while in bed. On or about May 28, 2025, the staff placed the AV in bed but failed to properly connect their tab alarm as care planned. Staff were not made aware the AV was attempting to get out of bed and suffered a fall, sustaining bruising and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
