Findings: The Alleged Victim (AV) is a fall risk with a history of falls. The AV is care planned for a pressure pad alarm on their bed and wheelchair. According to an investigation, on or about May 7th, 2025, it was identified the AV's pressure pad alarm was not functioning. On May 10th, 2025, the AV had two falls, placing them at risk of harm. The alarm was found in place at the time of the falls, but not working. The facility failed to ensure the AV's equipment was in good repair and functioning, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
