Findings: The Alleged Victim (AV) and Witness 1 (W1) have history of behaviors a previous resident-to-resident altercation with each other. It is known that Alleged Victim (AV) does not like having a roommate. According to an investigation, on or about June 30, 2025, staff heard yelling from the room that AV and W1 share and found W1 standing over AV. It was determined that W1 slapped AV, resulting in a red mark. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP25-01375 $188.00 fine assessed
