Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. On July 26, 2025, AV was able to open a facility door that was supposed to be locked and elope from the facility to an area very close to a busy road. The facility was aware that the doors were not locking correctly. The facility failed to provide a safe environment, which is neglect and constitutes abuse.
