Findings: The Alleged Victim (AV) relies on AP1 for safety. AV has declining memory, lack of orientation to place, time and situation. AV is known to follow staff and other residents around the facility. On or about September 9, 2025, Alleged Perpetrator 2 (AP2) went on break, AP2 did not notice AV following closely behind prior to opening the door, and AV came out right after AP2. AP1 had not provided elopement training to AP2 prior to AV eloping from AP1. The allegation that AP2 failed to provide a safe environment was investigated and the determination was not substantiated. The facility failed to provide a safe environment, and an appropriate training to AP2 around resident elopements, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
