Findings: Alleged Victim (AV) lives at Respondent’s facility. Around August 2025, facility staff reported Alleged Perpetrator #2(AP2) took pictures and videos of residents with AP2's personal phone. Including AV, AP2 took a picture of AV who was wearing only a brief. AP2's actions are a violation of resident rights, which is considered neglect of care and constitutes abuse. The facility failed to ensure the residents rights, which is a violation of Oregon Administrative rules.
