- Findings
- On September 11, 2025, Alleged Perpetrator 2 (AP2) assisted Alleged Victim (AV) outside of the facility in an attempt to diffuse behaviors AV was exhibiting. AP2 pushed AV in his/her wheelchair outside onto the raised sidewalk next to the parking lot. Based on facility documentation and interviews, AP2 left AV on the sidewalk to retrieve an item from AP2's car, failing to ensure the breaks on AV's wheelchair were properly locked before leaving AV unattended on the sidewalk which caused AV's wheelchair to roll toward the right side, in the direction of the sidewalk's edge. Both front wheels of AV's wheelchair rolled off the edge of the sidewalk, causing AV to propel out of his/her wheelchair in which AV landed on the parking lot pavement, resulting in AV suffering facial abrasions and a nasal hemorrhage. AV was sent to the hospital for treatment and was diagnosed with a facial fracture to the area below the left eye along with facial discolorations to the same area. AP2's actions caused a physical injury to AV which is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment for AV which a violation of Oregon Administrative Rules.
