Findings: On September 16, 2025, ODHS Adult Protective Services (APS) initiated investigation #00426844 and issued a written investigation report, which is incorporated here by reference. On September 15, 2025, the AV, a resident of the Respondent’s facility, eloped from the secured building without staff awareness. Based on facility documentation and interviews, AV had a documented history of elopement risk, as reflected in his/her care plan. In the days preceding the incident, AV had expressed intentions to leave the facility which were confirmed through staff interviews. Despite these warning signs, appropriate interventions were not implemented to mitigate AV's elopement risk. The facility failed to provide adequate supervision and monitoring, resulting in AV exiting the premises unsupervised which is a violation of resident rights, is considered neglect of care which constitutes abuse.
