Findings: The Alleged Victim (AV) relies on the facility for their care and has a history of falls, including a fall or about August 20, 2025. According to an investigation, after the previous fall, AV expressed complaints of knee pain on several occasions. On or about September 3, 2025, AV experienced another fall, resulting in daily complaints of knee pain and lack of desire to ambulate. AV was not seen by a medical provider until approximately September 8, 2025, where AV was diagnosed with a fracture to AV’s left knee. The facility failed to assure timely medical treatment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
