- The facility failed to protect the Alleged Victim (AV) from potential harm when the AV fell without injury on 9/28/2025. The failure is a violation of Oregon Administrative Rules. The allegation that Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) neglected AV by failing to follow the care plan was investigated and determined to be substantiated.
