Findings: The Alleged Victim (AV) is dependent on the facility to meet his/her safety needs and had a history of elopement behaviors. According to an investigation, on or about October 30, 2025, AV was found by law enforcement wandering in the community down the street from the facility. AV had been displaying exit seeking behaviors earlier in the day and was care planned to have safety checks eight times per shift. Facility staff were not aware of who was assigned to perform the safety checks or when safety checks had last been done. The facility failed to prevent elopement and follow interventions to ensure AV's service planned need for supervision. The facilities failure to follow the care plan resulted in risk of serious harm for AV, which is a violation of resident rights, is considered neglect of care which constitutes abuse.
