Findings: Beginning November 2024, Alleged Victim (AV) was a full assist for feeding per the service plan. Within the last two weeks, AV had been found in AV's apartment, with untouched, cold food. As a result, AV had lost weight recently from not eating. AV's care plan was not being fallowed, AV's television was being left on during meals times and staff were not always present for mealtimes. Respondent failed to follow the care plan, and to ensure the safety of AV. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
