Findings: The Alleged Victim (AV) relies on the facility to administer his/her medications. According to an investigation, on or about October 28, 2025, AV was administered an incorrect dose of anxiety medication on three occasions, resulting in risk of serious harm. Following a change in dose order on October 23, 2025, the narcotics book had not been updated and there was no medication audit conducted. The facility failed to provide a safe medication system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
