Findings: The Alleged Victim (AV) resides in the memory care, has a history of wandering behavior and is dependent on staff for daily care needs. Witness 1 (W1) has a history of sexual behaviors towards others. According to the investigation, on or about October 15, 2025, AV was found in W1's room and W1 had pants undone. It is believed W1 brought AV to their room as AV would not have been able to do this without assistance. W1's behaviors were being entered into a monitoring log, but the log had not been reviewed and no new interventions were implemented. This placed AV at risk of serious harm due to W1's history of sexualized behaviors. The facility failed to properly address resident behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
