Findings: Alleged Victim (AV) lived in the Respondent’s facility and depended on facility staff to ensure AV’s basic care and safety. AV’s service plan indicated that AV required facility staff assistance to manage medications and safely administer prescribed medications. On or about November 26, 2025, AV received the last dose of AV's mental health medication. AV did not receive prescribed medication until November 30, 2025, as it was documented as not available for administration. The Respondent failed to maintain a safe medication and treatment administration system and failed to administer medications as prescribed. This constitutes abuse by neglect as defined in OAR 411-020-0002(1)(b)(A)(ii).
