Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV's care planned fall interventions were that AV may require assistance during times of dizziness. According to an investigation, on or about August 31, 2024, AV reported to staff he/she was feeling dizzy and fell a few minutes later while left alone in h/h room, resulting in a hip fracture. The facility failed to provide appropriate care, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
