Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, AV was known by the facility to wander into other resident rooms, but the facility had no interventions in the service plan to address this. AV was found in Witness 1 (W1)'s room and suffered a cut on the back of their head after an altercation. The facility failed to implement interventions to AV's service plan for wandering, which is a violation of resident rights and is neglect which constitutes abuse.
