Findings: The Alleged Victim (AV) relies on the facility to provide services and a safe environment. On May 8, 2026, AV was able to exit the facility without the knowledge of facility staff. AV was found by individuals from a neighboring building who called the facility to alert them that AV was with them. This is when the facility was made aware that AV was missing from the facility. The facility failed to provide a safe environment for AV, which is a violation of resident rights and is neglect which constitutes abuse.
