Findings: Based on interview and record review, conducted during a site visit on 02/11/25, the facility's failure to develop and maintain an Acuity-Based Staffing Tool was substantiated. Findings include, but are not limited to: A review of the facility's posted staffing plan, undated, indicated the following: Day: two med techs and four caregivers; Evening: two med techs and four caregivers; and Night: one med tech and one caregiver. A review of the facility's schedule for Wednesday 12/25/24 revealed the facility was not staffing to their posted staffing plan. The following staff were scheduled: Day: one med tech and four caregivers; Evening: two med techs and three caregivers; and Night: one med tech and one caregiver. In an interview, Staff 1 agreed with the findings. The facility's failure to develop and maintain an Acuity-Based Staffing Tool was substantiated.
