Findings: Based on observation and interview, conducted during a site visit on 02/27/25, the facility’s failure to ensure locked storage for all poisons, chemicals, rodenticides, and other toxic materials was substantiated. Findings include, but are not limited to: Compliance Specialist (CS) observed a bottle labeled multi-purpose cleaner under the sink, in the kitchenette, in the Walnut neighborhood on 02/27/25. The gate to the kitchenette did not have a lock and was open. In an interview, Staff 2 (Caregiver) stated the door to the kitchenette didn’t lock, and chemicals were supposed to be kept in the locked storage next to the kitchen or in the storage down the hall. The facility’s failure to ensure locked storage for all poisons, chemicals, rodenticides, and other toxic materials was substantiated.
