Findings: On or about April 21, 2025, an investigation conducted by the ODHS Licensing Complaint Unit (LCU) determined that the facility failed to provide a safe medication administration system, as required under Oregon Administrative Rules. Based on a review of facility documentation and staff interviews, it was found that the facility did not have adequate systems in place to track and follow medical orders. This systemic failure resulted in missed or delayed administration of prescribed medications and treatments for residents, The facility failure is a violation of Oregon Administrative Rules.
