Findings: The facility failed to evaluate and monitor, and ensure a Registered Nurse assessed and documented findings for Resident #1 when he/she experienced a change of condition and safety issues. Resident #1 experienced significant weight loss and continued to lose weight. The failures are a violation of resident rights, are considered neglect of care and constitute abuse.
