- Findings
- The facility failed to exercise reasonable precautions in hygiene safety. Per complaint a staff member enters the facility without practicing hand hygiene, without a mask/eye protection, and with a school-aged child without a mask, whom is around. After conducting a compliance review it is noted that the allegation is substantiated.
